Transform Ageing aims to revolutionise the approach to health, wellbeing and social care for people in later life, starting in the south-west of England.
Bringing together people in later life, social entrepreneurs and commissioners of health and care services, Transform Ageing will define, develop and deliver new people-centred solutions that better support the needs and aspirations of older people.
The UK is undergoing a demographic shift with far-reaching consequences for the economy, public services and society. A vastly improved life expectancy, one of the great triumphs of the last century, looks set to be one of the great challenges of this one.
Right now there is a need to create a care system that is more personalised, more connected and above all more preventative. This requires new innovation and greater collaboration between individuals and carers, family and friends, neighbours, volunteers and professionals.
The goal is to create a replicable and sustainable new model for services across the UK. And to reimagine what it means to enjoy a good later life.
Transform Ageing ensures that we find the best innovations to change the lives of people in later life and that they reach them as quickly as possible.
The Transform Ageing team has been working to connect the needs and desires of local people in later-life, with the innovations and ideas of social entrepreneurs. We have developed six innovation briefs for ambitious social entrepreneurs to apply their creative expertise to in Somerset, Devon, Torbay and Cornwall.
People in later life have positive experiences of ageing
Enhancing independence and wellbeing through improved mobility
Supporting people to prepare for life changes
Supporting, valuing and celebrating carers
Making information accessible, relevant and meaningful
Creating opportunities for people in later life to connect
with other people, communities and activities
Each innovation brief was developed in collaboration with local people to capture their perspectives, experiences and insights into what will make a difference in their communities.
The briefs are designed to encourage social entrepreneurs from the south-west and around the UK to launch new services or products in the region, to directly meet the needs and desires of local people.

Can you help us tackle one of the innovation briefs? We’re looking for social entrepreneurs with ideas to transform the experience of ageing, whether that is the lives of people in later life, their family, friends or carers.
We’re looking for entrepreneurs with an idea as well as more established social ventures that are looking to scale their business. The programme will run in the south-west of England (Devon, Torbay, Cornwall and Somerset).
We are supporting early stage social entrepreneurs and those who are ready to scale:
Do It awards are designed to help you turn your ideas into real ventures. We provide 1-to-1 support, connections to specialist advisors and funding.
Grow It awards provide support to those who have proven their commitment and have successful ventures which are ready to grow or become self-sustaining.
Inspire awards are for local people improving the experience of ageing; sharing knowledge, experience and ideas with others to turn new ideas into action that will improve later life. Activity must be delivered in the SW in Devon, Torbay, Somerset or Cornwall and must benefit people in later life.
Scaling up awards are aimed at established social ventures, who are ready to scale up and require investment to do so. These social entrepreneurs are ready to further their impact by taking on grant funding, expert business support and potentially raising investment.
Up to £50,000 of grant funding is available alongside bespoke support and help to raise further investment. Match funding against risk capital is possible, but is arranged on a case-by-case basis.
Throughout the year we have three opportunities for social entrepreneurs to apply for an award. You can submit an expression of interest at any time.
Should your expression of interest pass our initial assessment you will be sent further application documents to complete along with a detailed breakdown of the application process.
